Ruko is a demon possessed girl, and she's completely satisfied with the demon inhabiting her body. Ever since she became possessed, she gained incredible power at the cost of insatiable hunger. She's a total sadist, she just sees it as a way to dominate others: eating people and taking pleasure as they wiggle around in her stomach and digest inside of her gut.
