Terraria Official Wiki: Plantera is a Hardmode boss that can be fought after all three Mechanical Bosses (The Destroyer, Skeletron Prime, and The Twins) have been defeated. Once this happens, Plantera's Bulbs will begin to spawn in the Underground Jungle biome.

My Description: Plantera is a malevolent jungle spirit who normally stays in hiding, planting her "Plantera Bulbs" to give herself 'respawn points', which also serve as bait to lure in prey. Getting close to one will give someone the itching desire to destroy it. She's around 12 feet (3.6 meters) tall and 10 feet (3 meters) wide. She will attack most creatures she sees intruding on her jungle, and will definitely kill/eat you if you destroy one of her bulbs. She's found her recent appearance into the nexus... interesting, but she quickly found a jungle to call home and set up shop there, making the jungle as it once was much more dangerous, causing new creatures and structures to appear out of nowhere, such as large hives, home to Queen Bee; shrines carrying untold treasures, and, of course, the Lizhard temple, home to the Golem. She cannot talk, save for growls and animal-like noises.

Defence: 36 (72 when Enraged/In her second form, 144 when Enraged and in her second form)

Abilities:
Plantera's Hooks - The ends of Plantera's vines, which cling to surfaces and deal 60 120 damage on contact.
Seeds (first stage) - She begins to fire seeds out of the tip of the bulb, doing 44 76 damage on hit. The green Poison Seeds are more deadly than the more frequently fired pink Seeds.
Thorn Balls (first stage) - These bounce around erratically through the Jungle and do 62 108 damage on contact. These can overwhelm the player if they stay in the same area for too long, as well as deal large amounts of damage on contact. Plantera will not begin releasing thorn balls until she drops below around 38000 life. She will stop releasing the thorn balls once she enters the second stage, although the thorn balls previously released may linger for a bit.
Plantera's Vines (second stage) - Gnawing minions on "leashes". They deal 60 120 damage and get in the way of the player's attacks, making piercing weapons more valuable. With 1000 2000 HP they are relatively weak and easily killed with proper equipment.
Spores (second stage) - She will shoot out pink spores that slowly float downwards. Easily destroyed, but deal 70 140 damage if touched.
Regeneration - If, or when, she dies, her body will regenerate from a Plantera Bulb in the jungle. The regeneration process takes 5 2 days at max.
Magic Resistance - She is resistant to most forms of magic, if you're too weak you will do literally nothing to her.

Phases:

First form
Plantera creeps after the player, firing pink Seeds and eventually green Poison Seeds; firing them more rapidly as she takes more damage. When her health nears the value at which she would change to her second form, she begins dispensing bouncing thorn balls.

Although Plantera emphasises her range, contact can still occur if she gets too close, causing 50 damage.

Second form
Once Plantera is brought down to half of her original health, she changes to her second form: Her pink petals fall away to reveal a thorn-filled mouth. In this form, Plantera moves more aggressively and attempts to make contact. She also sprouts additional mouths that are attached via vines (called "Plantera's Vines") and gains doubled defense, totaling it to 72.

She will also release floating pink spore projectiles that drift towards the player, dealing 140 damage on contact.

Enraged behavior
Plantera only becomes "enraged" if she follows the player out of the Underground Jungle. This causes her to obtain double damage, double defense (quadruple in the second form) and increased movement speed, as well as changing her attack patterns and directly rushing the player. Plantera's behavior returns to normal upon reentry of the Underground Jungle biome.

Expert Mode

In expert mode, there are a few significant differences in Plantera's attacks, aside from the default increased health and damage.

Phase 1
Plantera's seeds, that originally didn't travel through blocks, now travel through them and do increased damage. The thorn balls are also more common and home in on the player.

Phase 2
Plantera's vines, originally only attached to the body, now are attached to the Hooks and have increased in number (considering there are 3 more for each hook, there appear to be 9 extra vines). They also have more health than usual (including the default increased health). Plantera's body also does more damage and is faster than normal.

List of 'disappearences' within the Jungle:
Kyrii_Hatame - A harpy. She was an avid explorer, but she carelessly destroyed one of Plantera's bulbs. She was quickly dealt with.
Chester_ - A Saltwater Crocodile, who didn't realize how dangerous Plantera's bulbs could be. If he didn't have a reforming collar he wouldn't be alive to tell the tale.
Rose_Flora - A plant girl, specifically a rose. She destroyed one of Plantera's Bulbs in an attempt to stop the craziness in the jungle. She made the mistake of exiting the underground and was swallowed alive after being weakened.
Apprentice_Mia - A witch's apprentice, who cast a spell on one of Plantera's Bulbs, effectively destroying it, and went down and woke her from her slumber herself. Had she not used a reformation spell she would forever be plant food.
Lomi - A moth boy. Sucked some nectar from her bulbs, but accidently let a Hornet destroy the bulb. She made a meal out of him in no time.
MiniMickey - A bunny boy. Caved into the desire to destroy the plant. Now he's just a nutritious meal.
Camden - A suptype of shapeshifter, known as a Vorite. They were simply 'caught offguard' by a vine through the chest, which knocked out their core. Plantera, being the smart jungle creature she is, smashed it, killing Camden for good. (( done with permission of course ))
Updates:
When Blizzare and Gallent attempted to kill Plantera once and for all, she sent out a distress call to the Moon Lord, having him put the Nexus into Expert Mode and allowing her to become even more powerful.
